Prior to the Civil War, the United States’ economy was essentially agricultural based; slavery in the South was the key player in its prosperous economy. Hence, it is no wonder the South stood in defense of slavery’s permanence when challenged with the demand for abolition. The Southern proslavery Confederate states fought against the Northern antislavery Union states during the Civil War. The Union prevailed in the war and once the Confederates seceded and left the United States with a new predicament: The Reconstruction Era. In which, the Union needed to find a way to readmit the Confederates back into the nation in accord to Union principles. Essentially, although the ex-Confederates rejoined the Union, the reconstruction of the South was unsuccessful. Presidential, Congressional, and Radical reconstruction individually served to the collaborative failure of readmitting the South based on Northern ideals. Furthermore, Presidential Reconstruction was futile in the ref ormation of the South. During this phase of reconstruction, Abraham Lincoln proposed his Ten Percent Plan in which he strategized to readmit the South into the Union by having at least ten percent of the Confederate states vote back into the Union in exchange for amnesty so long as they accepted the thirteenth amendment which abolished slavery. Introduction To better understand what media sources the Singaporean diaspora uses to find out about news back home in Singapore, I conducted a survey amongst Singaporeans based overseas, namely in places such as Australia, the United States, some countries in South East Asia and also in Europe. The demographics used were age based, dispersed mainly among working class adults and university students. The survey resulted in a 40% readership of www.straitstimes.com, the online version of Singapore’s leading newspaper The Straits Times and a 27% readership of another website, which is also a subsidiary of the Straits Times, called Stomp. Answer: By reviewing the incident, there are several major reasons has been identified of this incident like structural issues, technical faults and most importantly communication gaps. First of all the design of the well construction must be standardized by two barriers to stop the uncontrollable flow of hydrocarbons. The primary pressure of these barriers should be cemented at the end of the casing to prevent the hydrocarbon flows from the reservoirs to the environment. Then the management of the company was also failed to guide the field employees to ensure that the operations were processed according to the documented requirements (Linde, Baram and Renn 2015). The secondary barrier which has been suggested for this particular case should be made with two PCCCS (Pressure-Containing Corrosion Caps) to design a constructed well. Some of the greatest problems which have been found out are errors during the installation of cement shoe which resulted in improper functionalities and removal of PCCCS for operational purposes. Blame was put on those people who were present at that time in the facilitated area but is the flawed decision of the operational personnel as they are directly engaged in these activities. People who are involved in the cementing activities also are not skilled enough to handle this situation with their technical abilities (Linde, Baram and Renn 2015). As per the organizational design, roles and responsibilities of the senior managers with construction engineers and drilling engineers played a significant role in the occurrence of this event due to serious gaps in their communication and skills. Solution As a solution to this incident, it can be recommended that organizational issues and risks must be incorporated into offshore safety rules and regulations. Risks must be identified with regulatory scrutiny process and should be documented properly so that higher authorities of the organization will be able to remove these problems from the grassroots level (Myrto Konstantinidou 2016).
